I am an in-network provider for many insurances please contact me with your insurance request.

If I am out-of-network for your insurance provider, please contact them and request Out-of-Network reimbursement. Be sure to ask if my services are covered. Upon request, I will provide a Superbill for submitting to your provider for Out-of-network reimbursement to you if they choose to do so. Please contact them to confirm and understand the procedure. 

These questions are important to ask your insurance company:

• How much of the fee is reimbursed for out-of-network providers within my plan?
• Have I met my deductible?                                                                                                  

For you to use insurance, all therapists must provide the insurance company with a mental health or substance use diagnosis determined through completing an assessment, generally done at your first session.  This diagnosis is determined using the criteria of the Diagnostic and  Statistical Manual of Mental Disorders – 5th edition.  There may be instances where you would prefer to not have a diagnosis on  health insurance records. If so, it would be best to use a payment method other than insurance.
